Guidelines for Completion:

Please log in with your EUID to use the webform.

Forms are due at least one week before your requested performance (no later that Friday evening the week before your desired date).

Faculty approval is required on all forms. The webform will automatically email to your private lesson instructor and Dr. Hall.

Forms that do not have all the information completed and accurate will not be accepted. This includes the year the composition was written as well as the birth and/or death year(s) of the composer. If your composer is living and you cannot find their birth date you can email them and request it. Also make sure you have the correct title of the piece listed.

Dr. Hall will determine performance order.
